Providing diabetes care coordination

Quantum Health worked to close care gaps by enhancing diabetes education. Every member has access to free nutritional counseling, testing supplies and digital health solutions. Personal Care Guide nurses work directly with members living with diabetes, helping them understand their A1c level and the importance of monitoring blood glucose daily. We help members set attainable goals, increase their health literacy and achieve better outcomes, engaging with 89% of all diabetic members.
