Experiencing an increase in chronic conditions among their 2,400 members, National Church Residences needed to improve engagement and help their members utilize available resources in a timely manner. Despite prevention’s critical role in maintaining wellbeing, members lacked understanding of the importance of preventive health measures. 

Solution

Encouraging usage of in-network providers

Noticing a significant decline in preventive care and an upsurge in chronic conditions, National Church Residences turned to us for communication support. Through our proprietary Real-Time Intercept® (RTI) technology, Care Coordinators were able to reach out and engage with members — as well as their providers — 120 days earlier than other industry models. Our dedicated team of Care Coordinators guided members toward in-network providers and appropriate care to proactively identify any potentially serious healthcare problems.
