It's more important than ever before in today's health care environment to get the right health education in front of care coordinators and members when they need it most. Recently, Quantum Health integrated the Healthwise® Care Management Solution into its proprietary technology platform PIVIT™ (Patient Information Virtual Integration Tool), to provide real-time healthcare education.
Two features of the Care Management Solution-Healthwise® Coach, a health education desktop, and Healthwise® Communicate, a secure member message center-provide Quantum Health's care coordinators with a single system to prescribe consistent health education to members. In addition, they can prescribe 168 shared-decision aids to help members make preference-sensitive care decisions in partnership with their doctor and care team.

Directly from Healthwise Coach, care coordinators can use the e-fulfillment option to easily send follow-up education to members through the Healthwise Communicate message center. This timely delivery means members are more likely to use the health education and take action. Care coordinators can also see the member's engagement, activity, and progress in Healthwise education programs.

Quantum Health is currently building a program on how to use reporting that includes member responses to the shared-decision aids. With this insight into a member's progress and activity, care coordinators can provide tailored follow-up and better guide members' care plans.
